The ACMA Annual Meeting is the nation's premier event for case management and transitions of care professionals.

Organizer
Founded in 1999, the American Case Management Association (ACMA) is the only professional membership organization created specifically for Hospital/Health System Case Managers. They provide quality resources, solutions and support for the practice of Case Management and Transitions of Care. Their national membership is composed of a variety of professionals including nurses, social workers and physicians, among others.
ACMA’s mission is to be the Association for Health Care Delivery System Case Management and Transitions of Care (TOC) Professionals. They aim to accomplish this mission by providing innovative professional development services and networking opportunities for their members, as well as by influencing the policies and laws related to the practice of case management.
